Physician vs. CAD Agreement: Concordance improved from 47.3% to 76.1% over nine months.
Xpert Referrals: Increased from 7.8% to 9.6%.
TB Detection Yield: Xpert-confirmed TB cases per 100,000 CXRs rose from 858 to 2112.
TB Detection: Physicians flagged 9.1% of CXRs as TB-presumptive, while CAD flagged 14.6%.
Xpert Positivity Rate: CAD + physician-flagged CXRs had a higher positivity rate (28.9%) vs. overall (19.3%).
Conclusion:
AI-assisted qXR 3.0 improved TB detection, increased physician-CAD agreement, and optimized Xpert testing referrals. CAD-supported screening enhanced case detection, making it a valuable tool for public health. Further threshold calibration is needed to refine sensitivity and specificity. Integration of qXR AI into TB screening can improve diagnostics in high-burden, low-resource settings.
